Healthcare support to care homes

Project overview

This initiative arose from the recognition that residential care homes had particularly high levels emergency care service use and high rates of hospital admissions. The project, which has been piloted in 15 care homes across Birmingham CCG areas, focuses on providing enhanced support to care home staff to try and increase in-house capacity to deal with healthcare needs of the residents, reduce the use of hospital services and lead to cost savings. Homes in the project pilot have received community nursing support, support from tissue viability nurses, pharmacists, dieticians, GPs and training on dementia awareness, end of life care and falls prevention. CLAHRCWM is providing advice to support Cross City CCG in carrying out their own evaluation of the project and making robust recommendations about the future extension of the initiative.
