Implementation of COPD Care Bundles in Rotherham and Doncaster

Project summary

In the last year of the current CLAHRC programme our COPD Theme is supporting the implementation of COPD Care bundles following acute hospital admission in two sites in South Yorkshire, The Rotherham NHS Foundation Trust (including BreathingSpace) and Doncaster Royal Infirmary. 

The COPD “discharge” bundle are a collective group of evidenced-based interventions to be delivered to patients admitted with acute exacerbation of COPD. 

These include:

  • Advice and referral to Smoking Cessation Services
  • Assessment and referral to Pulmonary Rehabilitation
  • Review of Inhaler Technique
  • Provision of COPD Information and contact name, number to use for advice during flare up
  • Arrangement of appropriate follow-up following discharge

The primary project aim is to provide consistent high quality care for COPD patients experiencing an acute exacerbation of their symptoms. Both sites are working collaboratively with frontline staff to improve the experience and delivery of care for COPD locally and capture new knowledge on implementing evidence-based practice.

We are working closely with the North West London CLAHRC team in sharing learning and using developed implementation tools from their previous bundle development work across many acute trusts in London. Both Rotherham and Doncaster project sites are developing project teams, plans and robust project measures and are linking this work with current or planned local CQUIN requirements.
