Improving the long-term management and outcomes for stroke survivors with multiple long-term conditions

Background

This project aims to develop, test and implement novel system-wide approaches to identifying, targeting and managing people with stroke who have multiple morbidities.

Summary

This project consists of a stakeholder engagement study to identify needs, solutions to address them and priorities to improve quality of long-term management for people with stroke and multi-morbidity; a data analytics study and analyses of needs and development of potential digital solutions for better management. They have linked two sets of data: The South London Stroke Register (SLSR), containing records of > 5,300 people with stroke (since 1995); and Lambeth DataNet (LDN), containing anonymised records from general practices in Lambeth (since 2006), including 2503 strokes.
